Registration for Youth Initiative 2010
1. Download the Registration Form. (You may need to install Adobe Reader if you do not already have it.)
2. Fill out the Registration Form in its entirety, answer the Short Answer Questions, and get all signatures
required.
3. Mail/Fax the Registration Form, Short Answer Questions, Liability Release, and $250 registration fee to:

4. All Registration Forms must be postmarked no later than May 15th, 2010.
Please note the following:
- Checks should be made out to "Youth Initiative"
- Registration Fees are non-refundable, but are transferable.

Who Can Register for YI?
- Rising 10th - 12th grade students are welcome to register. (All students must have completed the 9th grade.)
- Each church may send a maximum of 5 students plus one youth pastor/worker or chaperone.
- Each church may send a maximum of 5 students plus one youth pastor/worker or chaperone.
